Summary Despite many improvements in the TB work in the Republic of Karelia tuberculosis is still a great medical problem in the country. To be able to guarantee the sustainability of the achieved results and future improvements a prolongation of the cooperation with three more years is planned with emphasis on bacteriological diagnosis and improved cooperation between TB specialists and PHC sector in four pilot districts (Kem, Belomorsk, Pryazha, Olonets).
Background In the 1990's the tuberculosis situation in the Russian Federation deteriorated considerable. In the Republic of Karelia the TB incidence tripled and the mortality doubled. Filha has been cooperating with the Karelian TB Dispensary since 1999 in the fight against tuberculosis (I Phase 1999-2004, II Phase 2005-2007). The project has been evaluated twice. The second evaluation in 2007 recommended to prolong the project with an additional III Phase
Objectives The general objective of the project is to improve health of the Karelian people by reduced morbidity in TB, especially MDR-TB.
Specific objectives/indicators 1. Decrease in TB incidence 2. Decrease in MDR-TB incidence among new culture-positive cases
Results The III Phase of the project has two components: 1) Improved bacteriological diagnosis and 2) Improved cooperation between the TB sector and PHC in TB work. Expected results for component 1: a. Rapid and reliable microscopy, culture and drug susceptibility results b. Improved laboratory safety c. Laboratory network between Central TB laboratory and district microscopy laboratories established Expected results for component 2: a. Improved basic knowledge of TB and pulmonary differential diagnosis among doctors, feldschers, nurses and laboratory personnel in the PHC sector b. cooperation between the district TB services and local PHC concerning DOT for TB patients in continuation phase established c. standardised system for TB diagnosis in PHC established
