    Objectives To create safe working conditions and prevent nosocomial infections in hospitals it is needed to reconstruct the wards for treatment of MDR-TB in the Regional prison hospital for the intensive phase of treatment and Puksa prison hospital for the continuation phase of treatment.

    The reconstruction include improved sanitary conditions for the prisoners,- ventilationsystem with HEPA extractors,- cosmetic improvements,- electric works,- kanalization works etc.

    The wards for MDR-TB patients should be reconstructed as an infection control measure. The wards will be divided into an infectious zone for the MDR-TB patients and a non-infectious zone for the staff, separated by a sluse with negative airpressure in the infectious zone. The staff should wear 3M masks and special clothes when entering the infectious zone. The infectious zone will be divided into two entities separated by a sluse according to the Smear negative or positive status of the patient.

    Measures of infectious control, realized at present in the Regional UIN hospital , where TB cases are treated.

    1. Early detection of infectious TB cases and treatment with efficient chemotherapy.
    - Active case finding, examination by X-ray(fluorography) twice annually.
    - Early detection of infectious TB patients by direct smear microscopy.
    - Detection of infectious cases by clinical symptoms (with the help of X-ray).
    - Short treatment delay (DOT treatment).

    2. Administrative methods:
    - Organise separate patient wards for infectious TB cases
    - Isolation from other prisoners/sick patients untill sputum conversion
    - Collection of sputum for analysis is conducted in a separate room with natural ventilation
    - A well-timed exchange of information between the laboratory and medical personnel
    - Improved nutrition for tuberculosis patients
    - All the personnel within anti TB services is subject to obligatory medical examination, X-ray and doctor consultation
    - All medical personnel passes periodic physical examinations as person from the risk group of tuberculosis
    - Personnel, working with TB patients, have a 30- hour workweek, additional wage to compensate for dangerous health conditions of labour - 15 %. Personnel is provided with milk supplement for "harmth"- 0,5 litre in the day and additional vacation of 12 workdays
    - Medical personnel may have annual sanatorial- resort treatment or its money compensation and material aid (some extra money) is given for them to buy vitamins e.t.c

    3. Environmental level:
    - Simple measures are used for maximum perfection of the natural ventilations by openings of windows and opening external doors on opposite ends of the building
    - Bactericidal UV lamps are installed in the working rooms of the ward, such as in the room for the collection of sputum, the labs and procedure cabinets
    - Facilities for desinfection, containing chlorine, are used in accordance with acting normative documents of Ministry of Healthcare Russian Federation without harm to the environment

    4. Personal protective actions
    - Personnel of the department (physicians, nurses and sanitarian personnel) pass education in the preventive measures in tuberculosis and measures of individual protection
    - Medical personnel attend infection control courses at ARTD according to the joint plan of work and refreshing courses each five years

    Reconstruction of the MDR-TB ward as measure of infection control:
    1. Install mechanical ventilation with modern filtration method for removing exaust air from the rooms
    2. Division in "clean" and "infectious" areas of the department for MDR tuberculosis treatment
    3. Ventilation and installment of UV bactericidal lamps according to the graphics
    4. Installation of equipment and realization of engineering work by specialists of technical services from UIN
    5. Use of personal respiratory protection 3M 9320 FFP2S, at areas with increased risk of TB infection (in rooms for the collection of sputum, wards for MDR TB patients , in procedural and diagnostic cabinets). 6. Use of surgical masks by patients with infectious TB in order to prevent nosocomial transmission
    7. Ensure necessary amount of special clothes and footwears (masks, hubcaps, robes, shoes, gloves) for personnel
