Summary The project which is a health care developmental project in psychiatry has two interrelated goals:
1. Development of primary health care for patients with psychiatric problems
2. Differentiation and decentralisation of the specialized psychiatric care with a main focus upon patients with first episode psychotic disorders.

Sub goals are:
a) Define responsibilities, roles and routines for cooperation between primary care and the specialized psychiatric services.
b) Promote cooperation between patients, their families and the health services.
c) Promote multiprofessional cooperation within the different levels of health services.
Background The patients, who develop a severe mental disorder for the first time, meet all problems characteristic for Russian psychiatry in general: high level of stigmatization; low socioeconomic status; nonexistent public user organizations; almost nonexistent primary care practitioner who could diagnose the patients and provide treatment on out-patient level; mainly in-patient psychiatric services; insufficient multidisciplinary team work; low level budgets and bad physical conditions in the hospitals; very limited social support facilities in society: no secured housing, communal housing, specifically created/adapted workplaces/ conditions, etc.; there are no municipal units’ responsibility for the development such facilities. The responsibility for the mentally ill is placed on the county level. The consequence of this situation is that most families are alone in taking responsibility for their deceased family members when they are discharged from the hospitals.
Objectives Aim 1: Improvement of psychiatric competence on the primary care level
Aim 2: Establishing multidisciplinary intervention teams in the hospital and in out-patient clinics
Aim 3: Training and implementation of individually and group based psychotherapeutic interventions
Aim 4: Training and implementation of psychopharmacological treatment based on evidenced based principles and in monitoring the effects and adverse effects
Aim 5: The provision of adequate social rehabilitation
Aim 6: Training and implementing psycho educational family work
Aim 7: Evaluation of the effects of the new intervention model
Aim 8: Making the intervention program locally sustainable during a 5 years period
